Fighting insurance discrimination is important, and can be complicated. 
The Minnesota Department of Commerce can help.

Your story matters. Your complaint counts.

1. Start with the basics
Gather your policy information, and any paperwork that helps tell your story. You will need these to get going and get help. Write out your complaint, including how your experience affected your health and your life. Your story matters.

2. Get help with questions
Contact the Minnesota Department of Commerce. 
-  Call 651-539-1600 (800-657-3602, Greater MN-only)
-  Email KnowYourHealthInsuranceRights@state.mn.us.

3. Organize your information
You will need to know: ​
1. What type of policy you have (group, individual)
2. Insurance Policy Number
3. Claim Number
4. Date of Loss (from claim number)
5. What happened, when, and what was the result?

4. File your complaint
1. Go to mn.gov/commerce/consumers/file-a-complaint/
2. Create an account with your email and a password so
    you can login to the secure consumer portal.

3. Submit your complaint. Then you will get a Complaint
​    ID, and be asked to upload your paperwork. 

4. You may get an email from an investigator.
